Why this applies to Asenapine

Asenapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist for asenapine (Saphris, Secuado): your body partly relies on a liver enzyme called CYP1A2 to break it down and clear it out. Plumbagin, the active compound in Ceylon Leadwort, has been shown in lab dish studies to slow that same enzyme down. If it did the same thing in people, asenapine could clear more slowly and build up a bit, which might mean stronger effects like drowsiness, dizziness, lightheadedness when standing up, or numbness in the mouth.

The important part: this has only been seen in test-tube research, never in actual patients, so it's theoretical for now and not a reason to panic. Still, since asenapine affects your mood and alertness, it's worth a quick chat with your pharmacist or doctor before combining the two. Don't change anything on your own.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Asenapine Used For?

Brand names: Saphris, Secuado

Asenapine treats schizophrenia in adults and bipolar I disorder (manic or mixed episodes) in adults and in children ages 10 to 17. It can be used alone or alongside lithium or valproate in adults with bipolar I disorder.

The sublingual tablet forms (Saphris and the generic asenapine sublingual tablet) are approved for both schizophrenia and bipolar I disorder. Secuado, the skin patch, is approved only for schizophrenia in adults.
