Aspirin, Aluminum Hydroxide, Codeine Phosphate, Magnesium Hydroxide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

The piece to focus on here is the codeine in this combination product. Codeine is a prodrug, meaning your body has to convert it into morphine (the part that actually relieves pain) using a liver enzyme called CYP2D6. In lab tests, the plumbagin in Ceylon Leadwort appears to block that very enzyme. If it did the same in people, less codeine would turn into morphine, so you might get weaker pain relief than expected.

The good news: this is only test-tube evidence and hasn't been seen in real patients, so I wouldn't lose sleep over it. The aspirin, aluminum hydroxide, and magnesium hydroxide parts aren't really affected. Still, before adding this herb, run it by your pharmacist or doctor.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

The piece of this combination product that matters here is the aspirin. Aspirin is already a blood thinner that keeps your platelets from clumping together. Ceylon Leadwort may do something similar, since its root extract slowed platelet stickiness and lengthened bleeding time in animal studies. Stacked on top of aspirin, the theory is that you could bruise or bleed a little more easily, see longer bleeding from small cuts, or rarely notice stomach upset or bleeding.

I want to be honest about the strength of this: it is based only on lab and animal work, not on people, so it is a soft caution rather than a red alert. Still, since you are already on aspirin, it is worth running Ceylon Leadwort by your pharmacist or doctor before starting it.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
