The piece of this combo I want to focus on is the codeine. Codeine is actually a prodrug, meaning your body has to convert it using an enzyme called CYP2D6 to turn it into morphine, which is what actually relieves pain. Ceylon Leadwort contains plumbagin, which in lab testing slows down that same enzyme. So in theory, blocking CYP2D6 could mean less codeine gets activated, and you might get weaker pain relief than expected.
Here's the reassuring part: this is only based on test-tube research, not real people, so we don't know if it matters in practice. Still, if your pain isn't being controlled, that's worth mentioning. Please check with your pharmacist or doctor before pairing these two.

The piece of Fiorinal with Codeine that matters most here is the caffeine, which your body clears mainly through the CYP1A2 enzyme. In lab tests, plumbagin from Ceylon Leadwort can slow that enzyme down. If that happened in real life, caffeine could hang around longer and pile up, which might leave you feeling jittery, restless, with a racing heart, or having trouble sleeping. The aspirin, butalbital, and codeine pieces aren't really affected by this particular pathway.
I want to be honest about the strength here: this is only seen in test-tube studies, never reported in actual people, so it's mostly theoretical. Still, if you're using a concentrated supplement, it's worth a quick chat with your pharmacist or doctor before combining them.

Fiorinal with Codeine is a mix of four ingredients, and the one that matters most here is butalbital, the barbiturate that the body partly clears through the CYP3A4 enzyme. In lab dishes, the plumbagin in Ceylon Leadwort can slow CYP3A4 down. If that happened in a person, butalbital could build up a bit and add to the drowsiness, dizziness, and slowed breathing this product can already cause, especially since it also contains codeine, an opioid.
Here is the honest part: this is only a test-tube finding, never confirmed in people, so the real-world risk is uncertain. Still, this combo is already sedating, so I would run it past your pharmacist or doctor before pairing it with this supplement.

ANTICOAGULANT/ANTIPLATELET DRUGS

The piece of this combo pill that matters here is the aspirin. Aspirin already thins your blood a bit by making platelets less sticky, which is part of how it relieves pain and lowers clot risk. Ceylon Leadwort may do something similar. In animal studies, its root extract reduced platelet stickiness and made bleeding last longer, so stacking the two could, in theory, raise your bleeding risk.
What that might look like in real life: easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. I want to be honest though, this has only been seen in lab and animal research, not in people, so the concern is cautionary rather than proven. Before combining them, please check with your pharmacist or doctor.
