Aspirin, Codeine Phosphate, Methocarbamol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with this combo: the codeine part of Robaxisal C is a prodrug. Your body uses an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves your pain. Ceylon Leadwort contains plumbagin, which in lab studies appears to block that same enzyme.

So in this case, the worry isn't more side effects from codeine. It's the opposite: if that enzyme is slowed down, you might convert less codeine into morphine and get weaker pain relief. Keep in mind this is only test-tube evidence so far, never confirmed in real people, so the real-world effect may be small. The aspirin and methocarbamol pieces aren't really affected here. Still, run it by your pharmacist or doctor before combining them.

Aspirin, Codeine Phosphate, Methocarbamol also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

The combo pill you're asking about (Robaxisal C) contains aspirin, which is the piece that matters here. Aspirin already thins your blood a bit by keeping platelets from sticking together. Ceylon Leadwort may push in the same direction. In animal studies, its root extract reduced platelet stickiness and made bleeding last longer, so stacking the two could in theory raise your bleeding risk a little.

Realistically, that might show up as easier bruising, nosebleeds, bleeding gums, or a cut that takes longer to stop. Keep in mind this is based only on lab and animal research, not human studies, so the concern is more theoretical than proven. Still, I'd play it safe and check with your pharmacist or doctor before combining them, especially if you bleed easily or take other blood thinners.
