Ceylon Leadwort and Bellafoline, Phenobarbital

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES

Why this applies to Bellafoline, Phenobarbital

Bellafoline, Phenobarbital falls under CYTOCHROME P450 2B6 (CYP2B6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Belladenal product, which contains phenobarbital (a barbiturate sedative). One compound in Ceylon Leadwort, called plumbagin, was shown in lab dish studies to block an enzyme called CYP2B6, which is one of the pathways your body uses to clear phenobarbital. If that enzyme is slowed, phenobarbital could theoretically build up a bit higher than expected, leading to more drowsiness, dizziness, slowed breathing, or feeling overly sedated.

That said, this is only test-tube evidence and has never been seen in real people, so I wouldn't lose sleep over it. Still, phenobarbital is a drug where levels matter, so before adding Ceylon Leadwort, please run it by your pharmacist or doctor first.

From our pharmacist’s overview on the Bellafoline, Phenobarbital page

“Phenobarbital, the sedative half of Bellafoline (Belladenal), already slows the brain, so I treat this interaction list as one to respect rather than skim. More than half of the 141 entries, alcohol among them, add their own sedation on top of the drug's, which can deepen drowsiness and, in the worst pairings, suppress breathing.”

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain.
