Why this applies to Fedratinib Hydrochloride

Fedratinib Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Your body relies on a liver enzyme called CYP3A4 to break down fedratinib (Inrebic) and clear it out. Ceylon Leadwort contains a compound called plumbagin that, in lab tests, slowed this same enzyme down. If that happened in a person, fedratinib could build up more than expected, which may mean stronger side effects like nausea, diarrhea, low blood cell counts, or fatigue.

Here's the reassuring part: this concern comes only from test-tube research, not from real patients, so we honestly don't know if it matters in people. Still, fedratinib is a serious cancer medicine that already carries important warnings, so I wouldn't combine it with this supplement on your own. Please check with your oncology team or pharmacist first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
