These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Ceylon Leadwort interacts with whole categories of medication. Hydrochlorothiazide, Propranolol falls into 2 of those categories, so it can interact with Ceylon Leadwort in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Hydrochlorothiazide, Propranolol
Hydrochlorothiazide, Propranolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs a water pill (hydrochlorothiazide) with propranolol, a beta-blocker that helps lower blood pressure and slow the heart. Propranolol is partly cleared by a liver enzyme called CYP2D6. In lab dishes, plumbagin from Ceylon Leadwort appears to slow this same enzyme down. If that happened in a real person, propranolol could build up a bit more than usual.
In plain terms, that might mean stronger effects from the beta-blocker: a slower heart rate, low blood pressure, lightheadedness, or feeling unusually tired. I want to be honest though, this is only test-tube data, not something reported in people, so the real-world risk is uncertain. Please check with your pharmacist or doctor before adding this supplement, and don't change your medication on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
2 of 2Hydrochlorothiazide, Propranolol also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Your combo pill, Inderide, pairs hydrochlorothiazide (a water pill) with propranolol (a beta-blocker). The part that matters here is the propranolol, which your liver partly clears using an enzyme called CYP1A2. In lab dish studies, plumbagin from Ceylon Leadwort slowed that enzyme down. If the same thing happened in people, propranolol could build up a bit, which might lead to a slower heart rate, lower blood pressure than usual, dizziness, or feeling tired or lightheaded.
The honest part: this has only been seen in test tubes, not in real patients, so it stays theoretical for now. Still, since this is a heart and blood pressure medicine, I'd loop in your pharmacist or doctor before adding the supplement rather than guess.

From our pharmacist’s overview on the Hydrochlorothiazide, Propranolol page
“Inderide combines hydrochlorothiazide, a water pill, with propranolol, a beta blocker, so a supplement that also lowers blood pressure or pulls potassium out of the body hits the drug on its own turf. That describes most of this list, and I'd approach it with genuine care.”

About Ceylon Leadwort
What is Ceylon Leadwort?
- Digestive complaints and poor appetite
- Skin conditions and wounds (applied to the skin)
- Joint pain and swelling
- Traditional 'metabolism-boosting' tonics
- Antimicrobial folk remedy
Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.
