Why this applies to Hydrocodone

Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the tricky part with hydrocodone: it's a bit of a prodrug. Your body uses an enzyme called CYP2D6 to convert hydrocodone into hydromorphone, a more potent painkiller that adds to its pain-relieving punch. Ceylon Leadwort contains plumbagin, which in lab studies appears to block CYP2D6. If that blocking happened in real life, you might form less of that active compound, so hydrocodone could give you a bit less pain relief than expected.

The good news is that this is theoretical, based only on test-tube research, with nothing reported in actual people. So I wouldn't lose sleep over it. Still, before pairing this supplement with your hydrocodone, run it by your pharmacist or doctor so they can keep an eye on whether your pain stays well controlled.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the situation with Ceylon Leadwort and your hydrocodone. Hydrocodone leans on a liver enzyme called CYP3A4 to be broken down. In lab tests (not in actual people yet), plumbagin from this plant slowed that enzyme down. If that held true in the body, your hydrocodone could clear more slowly and build up a bit, which means stronger opioid effects than you expect.

For an opioid, that could look like more drowsiness, dizziness, constipation, slowed breathing, or feeling overly sedated. Keep in mind this is only a theoretical, test-tube finding, so I don't want you to panic. Still, opioids leave little room for surprises, so please check with your pharmacist or doctor before pairing the two.
