Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the wrinkle with hydrocodone: your body uses an enzyme called CYP2D6 to turn part of it into a stronger painkiller (hydromorphone). Ceylon Leadwort contains plumbagin, which in lab tests slowed that same enzyme down. If that happened in a person, you might actually get less of the strong active form, so your pain relief could feel weaker than expected.

The catch is that this is only test-tube evidence and hasn't been shown in real people, so I wouldn't lose sleep over it. Still, hydrocodone is a serious pain medicine, and how each person processes it varies. Before adding Ceylon Leadwort, please check in with your pharmacist or doctor so they can keep an eye on whether your pain stays controlled.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down hydrocodone. In test-tube studies, plumbagin (the main active compound in Ceylon Leadwort) appears to slow down that enzyme. If it did the same thing in people, hydrocodone could clear out of your system more slowly and build up higher than expected.

With an opioid, that could mean stronger than usual drowsiness, dizziness, constipation, and in a worst case slowed or shallow breathing. I want to be honest, though: this is only lab data, not something seen in real patients, so the risk is theoretical. Still, because hydrocodone is a powerful pain medicine, I'd loop in your pharmacist or doctor before combining the two rather than guessing.
