Why this applies to Lesinurad

Lesinurad falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for Lesinurad (Zurampic). Your body uses an enzyme called CYP2C9 to help break this medicine down. Ceylon Leadwort contains plumbagin, which in lab dish studies appears to slow that same enzyme. If it does that in real life, your Lesinurad could clear more slowly and build up a bit, which may raise the chance of side effects like kidney strain (Lesinurad already carries a kidney warning), headache, or reflux.

I want to be honest about the evidence though: this comes only from test-tube research, not from people, so it's theoretical right now. Since Lesinurad leans on the kidneys, I'd still play it safe. Please run this past your pharmacist or doctor before combining the two.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Lesinurad Used For?

  • Lesinurad is used in combination with a xanthine oxidase inhibitor to treat hyperuricemia (high levels of uric acid) in people with gout (sudden attacks of redness, swelling, pain, and heat in one or more joints) whose disease is not controlled with their current medication. Lesinurad is in a class of medications called selective uric acid reabsorption inhibitors. It works by helping the kidneys to remove uric acid from the body.
