Why this applies to Metformin, Rosiglitazone

Metformin, Rosiglitazone falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the thing to know about Avandamet, which combines metformin and rosiglitazone. The rosiglitazone part is partly cleared by a liver enzyme called CYP2C9. A compound in Ceylon Leadwort, called plumbagin, has slowed that enzyme down in lab dish studies only. If that held true in people, rosiglitazone could build up a bit, which in theory might push your blood sugar lower than expected (think shakiness, sweating, dizziness) or add to side effects like fluid retention and swelling.

The good news: this has never actually been reported in humans, so it's a theoretical concern, not a proven problem. Metformin itself isn't really affected this way. Still, before adding this supplement, check with your pharmacist or doctor and keep an eye on your blood sugar.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Avandamet combines two diabetes medicines, metformin and rosiglitazone, that work to lower your blood sugar. Here's the tricky part with Ceylon Leadwort: in animal studies, it has gone both ways, sometimes pushing blood sugar down and sometimes up. We haven't seen this play out in real people yet, so this is mostly a theoretical concern based on lab and animal work.

Still, if it nudged your sugar lower on top of Avandamet, you could feel shaky, sweaty, dizzy, or confused (signs of low blood sugar). If it worked the other way, your numbers might run higher than usual. Don't change anything on your own. If you want to try this supplement, check your sugar more often and talk with your pharmacist or doctor first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.
