Mirtazapine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for mirtazapine (Remeron): your liver uses an enzyme called CYP2D6 (among others) to help break this medication down. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to block that same enzyme. In theory, slowing that breakdown could let more mirtazapine build up in your body, which might mean stronger effects like extra drowsiness, increased appetite or weight gain, dry mouth, or feeling groggy.

The important caveat: this is only test-tube evidence, and it has never actually been reported in people. Mirtazapine also leans on other enzymes, so the real-world impact may be small. Still, it's worth a quick chat with your pharmacist or doctor before combining the two, especially if you notice more side effects.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist: your body uses several liver enzymes, including one called CYP1A2, to clear mirtazapine (Remeron) out of your system. Plumbagin, a compound in Ceylon Leadwort, has been shown to slow down CYP1A2 in lab tests. If that same effect happened in people, mirtazapine could clear a bit more slowly and build up, which might make its usual side effects stronger, things like next-day drowsiness, grogginess, increased appetite, or feeling dizzy.

Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

What Is Mirtazapine Used For?

Brand name: Remeron

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
