Why this applies to Nateglinide

Nateglinide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for Starlix (nateglinide). Your liver uses an enzyme called CYP2C9 to help clear this medication. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab tests to slow that same enzyme down. If it does the same thing in people, nateglinide could linger longer in your body and push your blood sugar lower than expected.

For a diabetes pill, that mainly means a bigger risk of low blood sugar (hypoglycemia), things like shakiness, sweating, dizziness, confusion, or a fast heartbeat. The honest part: this is only a theoretical concern based on test-tube studies, not real patients. Still, before you mix the two, check with your pharmacist or doctor so they can watch your sugars.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the honest picture with Ceylon Leadwort and nateglinide (Starlix). Nateglinide already lowers your blood sugar by nudging your pancreas to release insulin around meals. The catch is that Ceylon Leadwort has pulled blood sugar in both directions in animal studies, sometimes lowering it and sometimes raising it. So we can't predict cleanly which way it would push things alongside your medication.

If it adds to the effect, you could drift into low blood sugar (shakiness, sweating, dizziness, confusion). If it works against your medication, your sugar might run higher than usual. Keep in mind this comes from lab and animal research, not people, so it's theoretical for now. Please don't start or stop anything on your own, just check your glucose more closely and loop in your pharmacist or doctor first.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

Nateglinide tablets are used to help improve blood sugar (glucose) control in adults with type 2 diabetes. It is always used together with diet and exercise — not as a replacement for them.

Nateglinide is not for type 1 diabetes, and it is not used to treat diabetic ketoacidosis (a serious complication where the blood becomes dangerously acidic).
