Why this applies to Nintedanib Esylate

Nintedanib Esylate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the basic idea with Ofev (nintedanib) and Ceylon Leadwort. Your liver partly clears nintedanib using an enzyme called CYP3A4. A compound in this plant, plumbagin, has been shown to slow that enzyme down, but so far only in lab dish studies, not in actual people. So this is more of a theoretical concern than a proven problem.

If it did happen, blocking that enzyme could let nintedanib build up a bit higher than usual. With this drug, that tends to mean more of its common side effects, like diarrhea, nausea, vomiting, belly pain, and liver enzyme changes. Because the real-world risk isn't well established, I wouldn't panic, but please check with your pharmacist or doctor before adding this supplement rather than adjusting anything yourself.

Nintedanib Esylate also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's something worth knowing if you take Ofev (nintedanib). This medication carries its own bleeding risk, and our database flags Ceylon Leadwort as a supplement that may thin the blood too. In animal studies, leadwort root extract reduced platelet stickiness and made bleeding last longer. Stacking that on top of nintedanib could, in theory, raise your chance of bruising, nosebleeds, bleeding gums, or harder-to-stop cuts.

I want to be honest about the strength of this. The bleeding effect has only been seen in lab and animal research, not in people, so this is more a caution than a proven problem. Still, since nintedanib already affects bleeding, I'd play it safe and check with your pharmacist or doctor before adding Ceylon Leadwort.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Nintedanib

What Is Nintedanib Used For?

Brand name: Ofev

Nintedanib capsules are approved for adults with idiopathic pulmonary fibrosis (IPF) — a chronic, progressive scarring of the lungs — and for adults with chronic fibrosing interstitial lung diseases (ILDs) with a progressive phenotype, meaning fibrosing lung diseases that continue to worsen over time despite treatment.

Nintedanib capsules (including Ofev) are also indicated to slow the rate of decline in lung function in adults with systemic sclerosis-associated interstitial lung disease (SSc-ILD), a lung complication of the autoimmune condition systemic sclerosis (scleroderma). Note that the Nintedanib esylate capsule product is approved for IPF and chronic fibrosing ILDs with a progressive phenotype, but SSc-ILD approval is carried by Ofev and other nintedanib capsule products — always confirm with your prescriber which product you have been prescribed.
