Why this applies to Palbociclib

Palbociclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down and clear palbociclib (Ibrance). A compound in Ceylon Leadwort, plumbagin, has been shown to slow that same enzyme down, but only in lab dish experiments so far, not in real people. If it did the same thing in your body, palbociclib could build up to higher levels than intended.

With this drug, higher levels can mean more of its known side effects, especially low blood counts (your white cells, which fight infection), fatigue, mouth sores, and nausea. Because this is a serious cancer medicine and the evidence here is only theoretical, please don't start or stop anything on your own. Just run it by your oncology team or pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the honest picture with Ibrance (palbociclib) and Ceylon Leadwort. The concern on file is grouped under immunosuppressants, because the plant compound plumbagin has shown immune-calming activity in lab and animal studies. The thinking is that it might add to drugs that lower immune activity. Palbociclib isn't a classic immunosuppressant, but it does suppress bone marrow and can drop your white blood cell counts, so stacking another agent that nudges the immune system the same direction is theoretically not ideal.

I want to stress this is theoretical and based only on lab and animal work, not on people taking these two together. Still, since palbociclib already raises infection risk, I'd keep this supplement off the table until your oncologist or pharmacist signs off. Please don't change anything on your own.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
