Why this applies to Pazopanib

Pazopanib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Votrient (pazopanib). Your liver uses an enzyme called CYP3A4 to break this cancer medicine down. Plumbagin, the active compound in Ceylon Leadwort, has been shown in lab dish studies only to slow that same enzyme. If that effect carried over into the body, your pazopanib could clear more slowly and build up, which may mean stronger side effects like high blood pressure, diarrhea, nausea, fatigue, liver strain, or changes in your heart rhythm (QT prolongation).

I want to be honest though: this has never been reported in real people, so it's theoretical right now. Still, pazopanib is a serious drug with little room for error, so please don't add this supplement without first checking with your oncologist or pharmacist.

Pazopanib also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind with this pair. Pazopanib (Votrient) is a cancer medicine that can raise your bleeding risk on its own, and serious bleeding is one of its known warnings. Ceylon Leadwort may add to that. In animal studies, the root extract slowed platelets from sticking together and made bleeding last longer, which could stack on top of pazopanib's effect.

I want to be honest about the evidence, though. This concern comes from lab and animal work only, not from people, so it's more of a theoretical caution than a proven problem. Still, since pazopanib already carries a real bleeding risk, I'd play it safe. Watch for easy bruising, nosebleeds, bleeding gums, or blood in your stool, and please check with your oncologist or pharmacist before adding this supplement.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the honest picture with Ceylon Leadwort and pazopanib (Votrient). The concern on file is based on lab and animal work only, not on people taking these two together. In those studies, plumbagin (the main active compound in this plant) showed its own immune-dampening activity. The worry was that it might overlap with drugs given to suppress the immune system.

Pazopanib is a little different. It's a kinase inhibitor used to fight kidney cancer, not a classic immune-suppressing transplant drug, so this theoretical interaction is a loose fit. Still, we don't have good safety data combining the two, and plumbagin in concentrated supplement form can irritate tissues. I'd skip the supplement and run it by your oncologist or pharmacist first, especially since pazopanib already has serious effects to watch like liver strain and high blood pressure.

Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
