Why this applies to Temsirolimus

Temsirolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist with temsirolimus (Torisel). Your body relies on a liver enzyme called CYP3A4 to help clear this cancer drug. Plumbagin, a compound in Ceylon Leadwort, was shown in lab dish studies to slow that same enzyme down. If that happened in a real person, temsirolimus could build up to higher levels than intended, which may mean more side effects like mouth sores, rash, fatigue, low blood counts, high blood sugar, or raised cholesterol.

I want to be honest about the strength here: this is only test-tube evidence and has not been seen in actual patients. Still, temsirolimus is a serious chemo drug, so please don't add Ceylon Leadwort without first checking with your oncology team or pharmacist.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist for anyone using Temsirolimus (Torisel) for kidney cancer. This medicine works partly by quieting certain immune and growth signals (it's an mTOR inhibitor). Ceylon Leadwort contains plumbagin, which in lab and animal studies has shown its own immune-calming activity. The concern on file is theoretical: the two could pull on the same immune pathways at once, and we simply don't have solid human data to know how that plays out.

To be clear, the evidence here is weak (mostly test-tube and animal work), so I wouldn't lose sleep, but I also wouldn't add a concentrated supplement to a cancer regimen on my own. Since Temsirolimus is a serious drug with effects like infection risk, mouth sores, and lab changes, please run any Ceylon Leadwort use past your oncologist or pharmacist first.
