Tramadol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the tricky part with tramadol: it's actually a prodrug. Your liver uses an enzyme called CYP2D6 to turn it into the form that does most of the pain relieving. Ceylon Leadwort contains plumbagin, which in lab studies appears to block that same enzyme. So in theory, taking the two together could mean less of the active painkiller gets made, leaving you with weaker pain relief than you expect.

At the same time, the parent tramadol may build up, which can nudge up side effects like nausea, dizziness, or sleepiness. I'll be honest though, this is only seen in test-tube research, never confirmed in people, so it's more of a theoretical heads-up than a proven problem. Before combining them, please run it by your pharmacist or doctor.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the thing with tramadol: it's actually a partial prodrug. Your liver uses an enzyme called CYP2D6 to turn it into its stronger painkilling form, while another enzyme, CYP3A4, helps break it down along a different route. Ceylon Leadwort contains plumbagin, which in lab dishes appears to slow down CYP3A4. If that happened in people, more tramadol could linger in your system instead of being cleared.

In plain terms, you might feel more of tramadol's effects, like drowsiness, dizziness, nausea, constipation, or slowed breathing. But I want to be honest with you: this is only based on test-tube research, never confirmed in actual humans, so the real-world risk is uncertain. Still, since both can affect alertness, please check with your pharmacist or doctor before combining them.
