Aldesleukin falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the simple version: both of these can be hard on your liver, so putting them together is the concern. Aldesleukin (Proleukin) is a powerful immune-boosting cancer therapy, and liver problems (like raised liver enzymes and trouble with how your liver works) are already a known side effect of it. Epistane is a methylated anabolic steroid that is well known for stressing the liver too. Stacking two liver-stressors could, in theory, add up and make liver injury more likely.

I'll be honest, this warning is theoretical and based on how these drugs behave, not on real cases of the two together. Still, Aldesleukin is serious hospital-level treatment, and epistane isn't something I'd ever recommend. Please loop in your oncologist or pharmacist before mixing anything with it.

Proleukin (aldesleukin given by intravenous infusion) is approved to treat two serious cancers in adults: metastatic renal cell carcinoma (kidney cancer that has spread to other parts of the body) and metastatic melanoma (skin cancer that has spread). Both uses are specific to Proleukin administered intravenously in a hospital setting.
