Alectinib Hydrochloride falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the main worry with pairing these two: both can be hard on your liver. Alectinib (Alecensa) is well known to raise liver enzymes and, less often, cause more serious liver injury, which is why your oncology team checks bloodwork regularly during treatment. Epistane is a methylated anabolic steroid that is notoriously tough on the liver too. Stacking two liver stressors together could add up and make liver problems more likely.

To be fair, this concern is theoretical, based on how each drug behaves rather than reports of the two used together. Still, given that one of these is fighting cancer and the other is an illegal, risky steroid, I'd urge you to avoid epistane and talk with your oncologist or pharmacist before adding anything new while on alectinib.

What Is Alectinib Used For?

Brand name: Alecensa

Alecensa (alectinib) is used in adults with non-small cell lung cancer (NSCLC) whose tumors test positive for a gene change called ALK (anaplastic lymphoma kinase). Your tumor must be confirmed ALK-positive by an FDA-approved test before starting treatment.

Alecensa is approved for two situations: as adjuvant treatment (after surgery to remove the tumor, for tumors that are large enough or have spread to nearby lymph nodes) to help prevent the cancer from coming back, and for metastatic NSCLC (cancer that has already spread to other areas of the body).
