Methyltestosterone Buccal falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the thing to know: both of these are liver-stressful compounds. Methyltestosterone is a 17-alpha-alkylated steroid, which is the same chemical feature that makes it hard on your liver. Epistane is built the same way. So stacking them together means you're asking your liver to handle a double dose of that same kind of strain, and the worry is the harm could add up.

This is based on how these drugs behave chemically rather than a big pile of studies, so it's more of a theoretical caution. Still, real signs of liver trouble (yellowing skin or eyes, dark urine, belly pain, unusual tiredness, nausea) are worth taking seriously. Honestly, I'd steer clear of epistane entirely, and please loop in your doctor or pharmacist before combining anything with methyltestosterone.
