Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the gist with pomalidomide (Pomalyst): your liver partly clears it using an enzyme called CYP1A2. Ipriflavone is thought to slow that same enzyme down. So in theory, adding ipriflavone could let pomalidomide build up in your blood a little more than usual, which may mean stronger effects and more side effects.

With pomalidomide that could look like worse low blood counts (raising infection and bleeding risk), more fatigue, or a higher chance of blood clots. I want to be honest, though: this concern comes only from lab and test-tube data, not real patient studies, so it may not pan out in people. Given that pomalidomide is a serious cancer drug, please don't start ipriflavone without first checking with your oncology team or pharmacist.

Concurrent use of ipriflavone and immunosuppressant drugs might have additive effects on inhibiting immune function. Some patients taking ipriflavone experience subclinical lymphocytopenia; avoid concurrent use. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the practical concern with pairing ipriflavone and Pomalyst (pomalidomide). This isn't about one speeding up or slowing down the other in the liver. Instead, it's about adding two things together that can both push your immune system down. Pomalidomide is already well known for lowering blood counts, especially white blood cells (neutrophils), which raises infection risk. Ipriflavone has caused a mild drop in white cells (lymphocytopenia) in some people too.

Stacked together, those effects can add up, leaving you more vulnerable to infections and lower counts. Because pomalidomide is a serious cancer medicine that's already monitored closely with blood work, I'd play it safe here. Please check with your oncology team or pharmacist before adding ipriflavone rather than starting it on your own.

Ipriflavone is a synthetic isoflavone studied mainly for bone health and osteoporosis, but high-quality research has not shown that it reliably prevents fractures or builds strong bone. Because it can lower white blood cell counts in some people and its long-term safety is not well established, talk with your pharmacist or doctor before using it.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
