Dextromethorphan, Guaifenesin, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Of the three ingredients in this cough and cold combo, the one that matters here is dextromethorphan, the cough suppressant. Your liver uses an enzyme called CYP2D6 to clear it. Lab studies hint that lentinan might speed up that enzyme, which in theory could break dextromethorphan down a little faster and make it slightly less effective at quieting your cough. (Guaifenesin and pseudoephedrine aren't really cleared this way, so they shouldn't be affected.)

Here's the honest part: this idea comes only from test-tube and animal research, and it has never actually been seen in people. So I wouldn't lose sleep over it. If you take both and your cough just isn't easing up like you'd expect, mention it to your pharmacist or doctor.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for this cough-and-cold combo. Lab studies hint that lentinan may speed up a liver enzyme called CYP3A4, which helps break down the dextromethorphan part of this product. If that enzyme works faster, dextromethorphan could clear out a bit quicker, meaning your cough suppressant might not work quite as well. The guaifenesin (a mucus thinner) and pseudoephedrine (a decongestant) aren't really affected by this pathway.

Honestly, I wouldn't lose sleep over this one. It's only been seen in test tubes and animals, never in real people, and the effect is mild at most. Still, if your cough relief seems weaker than expected while using lentinan, mention it to your pharmacist before changing anything.
