Homatropine Methylbromide, Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with Hycodan: the hydrocodone part is actually a prodrug. Your body uses an enzyme called CYP2D6 to turn hydrocodone into hydromorphone, which does a lot of the pain-relieving and cough-quieting work. So if lentinan speeds that enzyme up, the way lab studies hint it might, you could end up converting it faster. In theory that could mean a bit more opioid effect for a while, things like extra drowsiness, dizziness, or slowed breathing, even though the drug clears out quicker overall.

I want to be honest about the evidence though: this has only been seen in test-tube and animal research, never confirmed in real people, so I wouldn't lose sleep over it. Just mention it to your pharmacist or doctor before pairing the two, especially since opioids deserve respect.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the situation with Hycodan or Tussigon, which pairs hydrocodone (a cough and pain medicine) with homatropine. The hydrocodone part is processed by a liver enzyme called CYP3A4. Lab research hints that lentinan might speed that enzyme up a bit, which in theory could clear hydrocodone a little faster and slightly soften its cough-calming or pain-relieving effect. It's not a one-to-one situation though, since hydrocodone is also turned into a more active form by a different enzyme (CYP2D6).

I want to be honest about the evidence here: this comes only from test-tube and animal studies, and it has never actually been seen in real patients. So I wouldn't lose sleep over it. Still, if you notice your cough medicine just isn't working like it used to, mention it to your pharmacist or doctor before changing anything.
