Why this applies to Rabeprazole

Rabeprazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Milk Thistle interacts with.

It is unclear if milk thistle inhibits CYP2C9; research is conflicting.
In vitro research suggests that milk thistle might inhibit CYP2C9. Additionally, 3 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP2C9 substrates, including imatinib and capecitabine. However, contradictory clinical research shows that milk thistle extract does not inhibit CYP2C9 or significantly affect levels of the CYP2C9 substrate tolbutamide. Differences in results could be due to differences in dosages or formulations utilized.

Here's the short version: there isn't much to worry about with milk thistle and rabeprazole. The theory is that milk thistle (silymarin) might slow down a liver enzyme called CYP2C9, which could let certain medicines build up. But the research is genuinely mixed, with some lab studies hinting at a small effect and good-quality human studies showing essentially none.

On top of that, rabeprazole leans more on a different enzyme (CYP2C19) and other pathways than on CYP2C9, so this particular interaction is a weak match for it. If anything tipped at all, you might see slightly more of rabeprazole's usual mild effects, like headache or stomach upset. This is rated minor and unlikely. Still, it never hurts to mention any supplement to your pharmacist so they can keep an eye on the whole picture.

It is unclear if milk thistle inhibits CYP3A4; research is conflicting.
While laboratory research shows conflicting results, pharmacokinetic research shows that taking milk thistle extract 420-1350 mg daily does not significantly affect the metabolism of the CYP3A4 substrates irinotecan, midazolam, or indinavir. However, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking milk thistle and cancer medications that are CYP3A4 substrates, including gefitinib, sorafenib, doxorubicin, and vincristine.

Here's the quick version: rabeprazole (Pariet) is mostly broken down by a non-enzyme pathway in your body, with the liver enzyme CYP3A4 playing only a smaller, secondary role. Milk thistle has been looked at as a possible blocker of that enzyme, but honestly the evidence is shaky and conflicting. In real people, milk thistle doses didn't meaningfully change how the body handled other CYP3A4 drugs.

So for rabeprazole specifically, any interaction is rated minor and unlikely. In theory, if milk thistle slowed that enzyme a bit, rabeprazole levels might tick up slightly, which could mean a touch more headache or stomach upset. That's not something I'd lose sleep over, but it's always worth a quick chat with your pharmacist or doctor before starting milk thistle.

About Rabeprazole

What Is Rabeprazole Used For?

Brand name: Pariet

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
