Duvelisib falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Ostarine interacts with.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist for this pairing. Duvelisib (Copiktra) can act as a mild blocker of an enzyme called CYP3A4, which is one of the same enzymes that helps clear Ostarine. In theory, slowing that enzyme could let a bit more Ostarine linger in your system, which might add to its effects and its risks, including the liver injury that Ostarine has been tied to.

That said, the real-world signal here is weak. When researchers paired Ostarine with itraconazole, a much stronger CYP3A4 blocker than Duvelisib, the Ostarine levels barely moved. So a meaningful interaction is unlikely. The bigger concern is Ostarine itself, which is unapproved, banned in sports, and risky next to a serious cancer therapy. Please loop in your oncology team before mixing anything with Duvelisib.

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
