Ritonavir falls under HEPATOTOXIC DRUGS — the group Ostarine interacts with.

Theoretically, concomitant use with hepatotoxic drugs might increase the risk of adverse hepatotoxic effects.
Some clinical research shows that ostarine can increase alanine aminotransferase, a marker of liver damage, in some patients. Additionally, the U.S. Food and Drug Administration warns that supplements containing SARMs, such as ostarine, have been associated with reports of liver toxicity and there are at least two reports of drug-induced liver injury attributed to the use of ostarine.

Here's the concern with pairing Ostarine and ritonavir (Norvir): both can be tough on your liver. Ritonavir is well known for sometimes raising liver enzymes and, less often, causing liver inflammation. Ostarine has the same reputation, with research showing it can bump up ALT (a blood marker of liver stress), and the FDA has tied SARMs like it to real cases of liver injury.

This isn't about one drug speeding up or blocking the other. It's that stacking two liver-stressing substances may add up and raise your odds of trouble. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. To be honest, I'd steer clear of Ostarine entirely (it's unapproved and banned in sports), but please talk this over with your doctor or pharmacist first.

Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS.

Theoretically, concomitant use of ostarine with CYP3A4 inhibitors could increase the effects and adverse effects of ostarine, although this is unlikely to be clinically significant.
Ostarine is partially metabolized by CYP3A4. However, clinical research shows that taking itraconazole, a potent inhibitor of CYP3A4, had minimal effects on the levels of ostarine in the body.

Here's the gist for this pair. Ritonavir is one of the strongest CYP3A4 blockers we have, and Ostarine is partly cleared by that same enzyme. On paper, slowing that breakdown could let more Ostarine build up, which in theory means a bigger chance of side effects like liver strain, which Ostarine is already known to cause.

The reassuring part: when researchers paired Ostarine with itraconazole (another powerful CYP3A4 blocker), the Ostarine levels barely moved. So even with ritonavir, a real-world problem is unlikely. That said, Ostarine is an unapproved SARM tied to liver injury and isn't recommended at all. If you're on ritonavir for HIV, please loop in your doctor or pharmacist before mixing anything like this in.

What Is Ritonavir Used For?

Brand name: Norvir

Ritonavir (brand name Norvir) is used together with other HIV medicines to treat HIV-1 infection in adults and children older than one month of age. It is always taken as part of a combination regimen — never on its own.

Norvir oral powder is specifically indicated for pediatric patients with HIV-1 infection. Norvir tablets and oral solution are indicated for both adults and pediatric patients with HIV-1 infection. Ritonavir tablets (generic) are also indicated for the treatment of HIV-1 infection in combination with other antiretroviral agents.
