Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's the piece to pay attention to with this combo product: one of its ingredients is aspirin, which already thins the blood by blocking platelets from clumping. Several ingredients in Puravive (ginseng, propolis, berberine from Phellodendron, kudzu) may have their own mild blood-thinning action. Stack them on top of aspirin and you could, in theory, nudge that effect a little further, meaning easier bruising, nosebleeds, or stomach upset and bleeding.

Honestly, the evidence here is weak, coming mostly from lab and animal studies, so I don't want to alarm you. The antacid parts (aluminum and magnesium) and codeine aren't really involved. Just give your pharmacist or doctor a heads-up before adding Puravive, especially if you bruise or bleed easily.

Cytochrome P450 2D6 (Cyp2D6) Substrates

Aluminum Hydroxide, Aspirin, Codeine Phosphate, Magnesium Hydroxide also falls under Cytochrome P450 2D6 (Cyp2D6) Substrates.

Puravive contains Panax ginseng. Some research suggests that Panax ginseng might slow down the activity of an enzyme called CYP2D6 by about 6%. This enzyme plays a role in breaking down drugs in our system. One study (https://pubmed.ncbi.nlm.nih.gov/32557706/) found that Panax ginseng slows down the breakdown of dextromethorphan, a drug that is processed by CYP2D6, but only to a small extent. It is important to note that not all studies agree on these effects, and some suggest that Panax ginseng might not have a significant impact on CYP2D6 at all. Nevertheless, it's important to be cautious if you're taking medications that are processed by CYP2D6 and consider discussing it with your healthcare provider.

The piece of this combo product that matters here is the codeine. Codeine is what we call a prodrug, meaning it does almost nothing on its own until your body converts it into morphine using the liver enzyme CYP2D6. Puravive contains Panax ginseng, which one study suggests may slow CYP2D6 by a tiny amount (around 6%). If that enzyme is nudged slower, your body would turn slightly less codeine into morphine, so in theory you might get a hair less pain relief.

Honestly, this is a very small and uncertain effect, and not every study even agrees it happens. A 6% change is unlikely to make a real difference for most people. The aspirin, aluminum hydroxide, and magnesium hydroxide parts are not affected. Still, if your pain control seems off, mention it to your pharmacist or doctor before changing anything.
