Treprostinil (Oral) falls under Anticoagulant/Antiplatelet Drugs — the group Puravive interacts with.

Puravive, a dietary supplement, contains several ingredients that may interact with anticoagulant drugs due to their potential mild anticoagulant effects. These ingredients include Kudzu, Panax ginseng, Phellodendron, and Propolis. Ginsenosides in Panax ginseng may decrease platelet aggregation in lab studies, but human studies have not confirmed this effect. Animal studies suggest low oral bioavailability of certain compounds in ginseng, which could explain differences between lab and human research. Propolis water extract and caffeic acid phenethyl ester, found in propolis, have also been shown to inhibit platelet aggregation in lab studies. Additionally, in an animal model, propolis taken with warfarin reduced the effectiveness of warfarin, suggesting a potential interaction with anticoagulant drugs. Lastly, Phellodendron contains berberine, which studies show may inhibit platelet aggregation.

Here's something worth knowing about oral treprostinil (Orenitram). On its own, this medicine relaxes and widens blood vessels to ease pulmonary arterial hypertension, but it also has a mild blood-thinning quality because it lowers how easily platelets clump together. A few ingredients in Puravive (like Panax ginseng, propolis, and the berberine in Phellodendron) may nudge platelets in that same direction, at least in lab studies.

Stacked together, the theoretical worry is a bit more bruising or bleeding, such as nosebleeds, easy bruises, or bleeding gums. I'll be honest, though: the human evidence here is weak and based mostly on test-tube work, so this is more a 'keep an eye out' than a red alarm. Please run it past your pharmacist or doctor before combining them.

Here's the gist for oral treprostinil (Orenitram), which widens blood vessels to ease the high pressure in your lungs. One ingredient in Puravive, quercetin, has shown a mild blood-pressure-lowering effect in a small study of people with mild hypertension. So in theory, pairing it with treprostinil could nudge your blood pressure a little lower than expected.

If that happened, you might notice more of treprostinil's usual effects like lightheadedness, headache, flushing, or feeling dizzy when you stand up. Honestly, this is a weak, theoretical concern, and food amounts of quercetin are very unlikely to matter. Still, treprostinil is a serious medication, so chat with your pharmacist or doctor before adding Puravive, and don't change your dose on your own.

About Treprostinil

What Is Treprostinil Used For?

Brand names: Remodulin, Orenitram

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
