Darunavir, Cobicistat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for your Prezcobix (darunavir with cobicistat). Darunavir is cleared by a liver enzyme called CYP3A4, and lab dish studies hint that red clover's plant estrogens could slow that enzyme down a touch. In theory, slowing it could let darunavir levels creep up, which might mean more side effects like nausea, diarrhea, headache, or rash.

But I want to keep this in perspective. When researchers actually tested red clover in people using a CYP3A4 test drug, they saw no real effect on how the body handled it. So this concern is mostly theoretical and rated as minor and unlikely. Since your HIV therapy is too important to gamble with, just give your pharmacist or doctor a heads-up before adding red clover so they can keep an eye on things.

About Darunavir and Cobicistat

What Is Darunavir and Cobicistat Used For?

Brand name: Prezcobix

Prezcobix and Prezcobix Ped are used to treat HIV-1 (the virus that causes AIDS) in adults and children 3 years of age and older who weigh at least 15 kg (about 33 lbs). They are always used together with other HIV medications — never alone. They are approved for people who are new to HIV treatment (treatment-naïve) as well as those who have taken HIV medicines before (treatment-experienced), as long as their virus does not have certain darunavir resistance mutations.

Prezcobix is specifically approved for adults and pediatric patients weighing at least 40 kg. Prezcobix Ped is available for smaller pediatric patients aged 3 and older weighing at least 15 kg. The right product and dose depend on the patient's weight and their ability to swallow tablets.
