Sirolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: your body clears sirolimus using a liver enzyme called CYP3A4. In a test tube, red clover's plant estrogens look like they could slow that enzyme down, which in theory might let sirolimus build up. Since sirolimus is a transplant medication with a narrow safe range, higher levels could mean more side effects like mouth sores, swelling, high cholesterol, or low blood counts.

The reassuring part: this is mostly a lab-based worry. A real study in people found red clover did not meaningfully change levels of a similar CYP3A4 drug, so an actual problem is unlikely. Still, because your sirolimus levels are closely monitored, please run any red clover supplement past your transplant team or pharmacist first.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. What Is Sirolimus Used For?

Brand name: Rapamune

Sirolimus oral solution and tablets are used to help prevent organ rejection in kidney (renal) transplant patients aged 13 and older. They are also used to treat lymphangioleiomyomatosis (LAM) — a rare lung disease in which abnormal muscle-like cells invade lung tissue.

Fyarro (sirolimus, intravenous injection) is used to treat adults with locally advanced, unresectable, or metastatic malignant PEComa — a rare tumor that grows in soft tissue or organs. Hyftor (sirolimus topical gel) is used to treat facial angiofibroma (small benign skin growths on the face) associated with tuberous sclerosis in adults and children aged 6 and older.
