Acetaminophen, Ibuprofen falls under GLUCURONIDATED DRUGS — the group Uva Ursi interacts with.

Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.

Here's the short version: this is a very theoretical concern, not something we've seen play out in real people. Uva ursi blocked a liver enzyme called UGT1A1 in a test tube, and your body partly clears acetaminophen through that glucuronidation pathway. On paper, that could let acetaminophen levels run a little higher. But the same effect did NOT show up in animal studies, and it has never been reported in humans, so I wouldn't lose sleep over it.

Neither acetaminophen nor ibuprofen is a prodrug, so we're not talking about a stronger pain-relieving punch here. If anything theoretical happened, it would lean toward more acetaminophen in your system. Since acetaminophen is hard on the liver in big doses, and uva ursi itself can stress the liver, keep uva ursi short-term and check with your pharmacist first.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine.
