Why this applies to Boceprevir

Boceprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea with these two together. Your liver uses an enzyme called CYP3A4 to help clear boceprevir (Victrelis) from your body. In lab tests, uva ursi seems to slow that same enzyme down. If that happened in real life, more boceprevir could hang around, which might mean stronger side effects like fatigue, nausea, a metallic taste, or worsening of the low blood counts (anemia) that already come with hepatitis C treatment.

The important catch: this has only been seen in the lab, not in actual people, so it's a theoretical concern rather than a proven problem. Still, since hepatitis C therapy is serious business, I'd skip the uva ursi and check with your pharmacist or doctor before adding any herbal product.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the short version: boceprevir is moved around your body in part by a transporter pump called P-glycoprotein. In lab dish studies, uva ursi seemed to slow that pump down. If that happened in people, a little more boceprevir could stay in your system, which in theory might mean a bit more of its usual side effects, things like a metallic taste, nausea, tiredness, or anemia (low red blood cells).

The honest part is that this is only seen in test tube and animal work, not in real patients, so it's a weak, mostly theoretical concern. Still, since you're treating hepatitis C and your liver is already working hard, and high doses of uva ursi can stress the liver, please run this by your pharmacist or doctor before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Avoid it during pregnancy and breastfeeding.
