Letrozole, Ribociclib Succinate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist for your Kisqali plus Femara combo. Both letrozole and ribociclib are cleared from the body partly by a liver enzyme called CYP3A4. In lab dishes, uva ursi seems to slow that same enzyme down. If that held true in people, it could let these medicines build up a bit higher than expected, which might mean more of the usual side effects like fatigue, nausea, low blood counts, or for ribociclib, changes in your heart's rhythm (QT prolongation).

The honest part: this is only a test-tube finding and has never been confirmed in actual patients, so it's theoretical. Still, ribociclib is a sensitive drug, so I wouldn't add uva ursi without first running it past your oncologist or pharmacist.
