Mometasone, formoterol falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.

Here's the honest picture with Zenhale (mometasone plus formoterol) and uva ursi. The concern on file is that uva ursi might slow down a liver enzyme called CYP2C19. In theory, slowing that enzyme could let a medicine that depends on it build up a bit. The catch is that this was only seen in a lab dish, never in real people, so it's a very soft, theoretical signal.

On top of that, your inhaled Zenhale acts mostly right in your lungs and very little reaches the bloodstream, so a small liver enzyme change is unlikely to cause real trouble. If you did notice more jitteriness, a faster or pounding heartbeat, or shakiness, mention it. Since uva ursi can stress the liver and is only meant for short-term use, please check with your pharmacist or doctor before combining them.

Here's the short version on Zenhale (mometasone with formoterol) and uva ursi. The mometasone part is partly cleared by a liver enzyme called CYP3A4. In a lab dish, uva ursi seemed to slow that enzyme down. If that happened in real life, your body might break mometasone down a little slower, leaving a touch more of it around. With an inhaled steroid that could mean things like throat irritation, hoarseness, or thrush (a white mouth infection), and rarely steroid-related effects if levels climbed.

The big thing to know: this is only a test-tube finding and has not been seen in actual people, so I wouldn't lose sleep over it. Still, since uva ursi is meant for short-term use only, it's worth a quick chat with your pharmacist or doctor before combining them.
