Perampanel falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.

Here's the gist for Perampanel (Fycompa). Lab studies suggest uva ursi may slow down an enzyme called CYP2C19, which is one of the pathways your body uses to clear perampanel. If that enzyme is blocked, perampanel could build up a bit more than usual, which might mean stronger effects and more side effects like dizziness, drowsiness, trouble walking, or mood changes such as irritability or anger.

I want to be honest though: this concern comes only from test-tube data, not from real people, so it's theoretical at this point. Uva ursi is also meant for short-term use only. Since perampanel is a seizure medicine, please don't change anything on your own. Just check with your pharmacist or doctor before pairing the two.

Here's the basic idea. Your body relies on a liver enzyme called CYP3A4 to clear perampanel out of your system. In lab studies (test tube only, not in actual people), uva ursi seemed to slow that same enzyme down. If that were to happen in the body, perampanel could build up a bit higher than expected. With perampanel, more drug usually means more of its known effects, things like drowsiness, dizziness, trouble with balance, and sometimes mood or irritability changes.

I want to be honest about the evidence, though. This is theoretical and has not been shown in humans, so it may not amount to much in real life. Still, since perampanel already causes sleepiness, it's worth a quick chat with your pharmacist or doctor before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
