Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist with Adempas (riociguat) and uva ursi. Your liver uses an enzyme called CYP3A4 to help clear riociguat from your body. In lab studies, uva ursi appears to slow that enzyme down. If that were to happen in real life, riociguat could build up a bit more than expected, which might lead to stronger effects like low blood pressure, dizziness, lightheadedness, headache, or an upset stomach.

I want to be honest about the strength here: this is only seen in test-tube studies, not in actual people, so it's more of a theoretical concern than a proven problem. Still, riociguat is a medicine where blood pressure changes matter. I'd just give your pharmacist or doctor a quick heads-up before pairing the two.

Here's the gist for Riociguat (Adempas): it's one of the medicines moved around the body by a transporter called P-glycoprotein. In a lab dish, uva ursi seemed to slow that transporter down. If that happened in people, slightly more riociguat could stay in your system, which in theory might nudge up its effects, things like low blood pressure, dizziness, headache, or feeling lightheaded.

I want to be honest, though: this is only a test-tube finding and has not been seen in real patients, so it's a minor, theoretical concern. Since riociguat is a medication where blood pressure matters, it's still worth a quick mention to your pharmacist or doctor before adding uva ursi, especially long-term.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
