Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the basic idea with uva ursi and Selinexor (Xpovio). Your body relies on a liver enzyme called CYP3A4 to help clear Selinexor. In lab-dish studies, uva ursi seemed to slow that enzyme down. If that were to happen in real life, Selinexor could build up a bit more than expected, which might mean stronger side effects like nausea, vomiting, poor appetite, fatigue, or low blood counts.

I want to be honest about the evidence here, though. This is only a theoretical, test-tube finding, and it hasn't actually been shown in people. Still, since Selinexor is a serious cancer medicine, I'd play it safe. Please loop in your oncology team or pharmacist before adding uva ursi, rather than starting or stopping anything on your own.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Selinexor Used For?

Brand name: Xpovio

Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.

Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
