Selumetinib Sulfate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist with Koselugo (selumetinib): your child's body relies on a liver enzyme called CYP3A4 to clear this medicine. Uva ursi has shown, in lab dish studies only, that it might block that same enzyme. If that were to happen in real life, selumetinib could build up a bit higher than expected, which may bring on more of its known side effects like nausea, vomiting, diarrhea, mouth sores, rash, or tiredness.

I want to be honest about the strength of this: it's theoretical and hasn't been seen in actual people, so the real-world risk is uncertain. Still, selumetinib is a serious cancer-type medicine for a vulnerable group, so please don't add uva ursi without first checking with your oncology team or pharmacist.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
