Why Are There 2 Interactions?
Bitter Orange interacts with whole categories of medication. Modafinil falls into 2 of those categories, so it can interact with Bitter Orange in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Bitter Orange interacts with.
Small clinical studies suggest that single or multiple doses of freshly squeezed bitter orange juice 200-240 mL can inhibit CYP3A4 metabolism of drugs, causing increased drug levels and potentially increasing the risk of adverse effects. However, the extent of the effect of bitter orange on CYP3A4-mediated drug interactions is unknown. Some evidence suggests that bitter orange selectively inhibits intestinal CYP3A4, but not hepatic CYP3A4. Its effect on P-glycoprotein, which strongly overlaps with CYP3A4 interactions, is unclear. One small clinical study shows that drinking 8 ounces of freshly squeezed bitter orange juice has no effect on cyclosporine, which seems to be more dependent on hepatic CYP3A4 and P-glycoprotein than intestinal CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two together. Your body partly uses an enzyme called CYP3A4 to clear modafinil. Bitter orange supplements can block some of that enzyme (mainly in the gut), which could let a little more modafinil hang around than usual. Modafinil isn't a prodrug, so more drug means stronger effects, things like a racing heart, jitteriness, headache, trouble sleeping, or higher blood pressure.
On top of that, bitter orange is itself a stimulant, so stacking the two could add up. The evidence here is modest and the real-world size of the effect isn't well known, so I'm not trying to alarm you. Still, since both can rev up your heart, please check with your pharmacist or doctor before pairing them, especially if you have blood pressure or heart concerns.

Here's the simple version: both bitter orange and modafinil are stimulants. Bitter orange contains synephrine, which can nudge your heart rate and blood pressure up. Modafinil, used to keep you awake for narcolepsy, sleep apnea, or shift work, can do something similar in some people. Stack the two together and those effects can add up.
What that might feel like is a faster or pounding heartbeat, jitteriness, higher blood pressure, headache, or trouble sleeping. This is mostly a theoretical concern, so don't panic, but if you have any heart or blood pressure issues it's worth taking seriously. Modafinil isn't activated by bitter orange, so this is just two stimulants overlapping. Please check with your pharmacist or doctor before combining them.

Bitter orange is a citrus fruit whose extracts contain synephrine, a mild stimulant often added to weight-loss and energy supplements. Evidence that it works for weight loss or performance is limited, and because it can raise heart rate and blood pressure, people with heart problems, high blood pressure, or who are pregnant should avoid it. What Is Modafinil Used For?

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).
In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
