Aspirin, Caffeine, Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Here's the wrinkle with this combo: the codeine in your Anacin product is a prodrug. Your body relies on an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. A compound in Ceylon Leadwort (plumbagin) has been shown in lab studies only to block CYP2D6. If that happened in people, you could convert less codeine into morphine, meaning weaker pain relief rather than stronger.
Keep in mind this is theoretical and has not been reported in humans, so I wouldn't lose sleep over it. The aspirin and caffeine pieces aren't really affected here. Still, before pairing this supplement with your medicine, it's worth a quick chat with your pharmacist or doctor.

In this combo pill, the piece that matters here is the caffeine. Caffeine is cleared by an enzyme in your liver called CYP1A2, and a compound in Ceylon Leadwort (plumbagin) has slowed that enzyme down in lab dish studies. The idea is that if the enzyme works slower, caffeine could hang around longer and build up.
If that happened, you might feel the usual too-much-caffeine effects: jitteriness, a racing or pounding heart, trouble sleeping, or an upset stomach. Honestly, though, this is only a theoretical concern right now, seen in test tubes and not in real people, so I wouldn't lose sleep over it. The aspirin and codeine parts aren't driven by this pathway. Just give your pharmacist or doctor a heads-up before pairing them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Here's the wrinkle with this combo pill. The codeine part is a prodrug, meaning your body has to convert it into morphine before it relieves pain. That conversion runs mostly through a different enzyme (CYP2D6), not CYP3A4, so the lab finding here doesn't cleanly tell us codeine will be stronger or weaker. CYP3A4 actually helps clear codeine down a side path, so blocking it might nudge a little more toward morphine and slightly raise the chance of drowsiness, constipation, or nausea.
That said, this is only a test-tube finding (plumbagin blocking CYP3A4), never shown in real people, so I wouldn't lose sleep over it. The aspirin and caffeine pieces aren't really driven by this enzyme. Still, please run it by your pharmacist or doctor first.

ANTICOAGULANT/ANTIPLATELET DRUGS

The piece of this combo pill that matters here is the aspirin. Aspirin already thins the blood by making your platelets less sticky, which is exactly the same effect that Ceylon Leadwort showed in early animal studies. Stacking the two together could, in theory, push that blood-thinning effect a bit further and raise your chance of bruising, nosebleeds, or stomach bleeding.
I want to be honest about the evidence though. This concern comes only from lab and animal research, and it has not been shown in people, so I wouldn't lose sleep over it. Still, since aspirin is the variable here, it's worth running this past your pharmacist or doctor before adding the supplement, especially if you ever notice easy bruising or dark stools.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts.
