Bisoprolol, Hydrochlorothiazide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist for your Ziac, which combines bisoprolol (a beta-blocker) with hydrochlorothiazide (a water pill). Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dishes to slow down an enzyme called CYP2D6. Bisoprolol is partly handled by this enzyme, so in theory, blocking it could let more of the drug build up in your system.

If that happened, you might notice a slower heart rate, lower blood pressure, dizziness, or feeling unusually tired. I want to be honest though: this is only seen in test-tube studies, not in real people, so it's a theoretical concern, not a proven one. Bisoprolol also clears mostly through other routes, which softens the worry. Still, since these are heart and blood pressure meds, it's worth a quick chat with your pharmacist or doctor before starting this supplement.
