Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist: your body relies partly on an enzyme called CYP2D6 to break down and clear clomipramine. A compound in Ceylon Leadwort called plumbagin appears to block that enzyme, at least in lab dish studies. If that same blocking happened in people, clomipramine could clear more slowly and build up in your system.

Clomipramine is not a prodrug, so higher levels would mean stronger effects and a greater chance of side effects like drowsiness, dry mouth, constipation, dizziness, a racing heartbeat, or heart rhythm changes. The reassuring part: this is only a theoretical concern based on test-tube work, never reported in real patients. Still, since this drug needs careful dosing, please check with your pharmacist or doctor before combining the two.

Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist with Ceylon Leadwort and your clomipramine. Your body relies on a liver enzyme called CYP1A2 to help break clomipramine down. In lab dish studies, plumbagin (the main active compound in this plant) appears to slow that enzyme. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).
