Desipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down desipramine and keep its level in a safe range. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dishes to slow that enzyme down. If that happened in a real person, less of the drug would get cleared, so desipramine could build up. With a tricyclic like this one, higher levels can mean more dry mouth, constipation, drowsiness, dizziness on standing, a racing heartbeat, and in rarer cases heart rhythm changes.

The important caveat: this is only test-tube evidence and has never been reported in actual patients, so the real-world risk is unknown. Still, desipramine has a fairly narrow safety window, so I'd run any Ceylon Leadwort use by your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the gist: your body partly relies on a liver enzyme called CYP1A2 to clear desipramine (Norpramin). Ceylon Leadwort contains a compound called plumbagin that, in test-tube studies, slows that enzyme down. If that same effect happened in people, desipramine could build up higher than usual in your system. Desipramine is an active drug, not a prodrug, so more of it means stronger effects, things like dry mouth, constipation, drowsiness, dizziness, a fast or irregular heartbeat, and in larger amounts, heart rhythm changes.

The important caveat: this has only been seen in the lab, never confirmed in humans, so it's theoretical. Still, since tricyclics can be touchy, I'd loop in your pharmacist or doctor before combining them rather than guessing.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Brand names: Norpramin, Desipramine, Pertofrane

Desipramine hydrochloride tablets — sold under the brand names Norpramin, Desipramine HCL, and Desipramine Hydrochloride — are FDA-approved for the treatment of depression in adults. Norpramin tablets and the generic formulations share this same approved indication.

Desipramine tablets are not approved for use in children. Adolescents and elderly patients may be prescribed it in certain circumstances, but at lower doses and under close supervision.
