Ceylon Leadwort and Fluphenazine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Fluphenazine

Fluphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body relies on a liver enzyme called CYP2D6 to break down and clear fluphenazine. Ceylon Leadwort contains plumbagin, which in lab dish studies appears to slow that same enzyme down. If it does the same thing in people, fluphenazine could clear more slowly and build up in your system, which means stronger effects and a higher chance of side effects.

With an antipsychotic like fluphenazine, that could show up as more drowsiness, dizziness, low blood pressure, or muscle stiffness and tremors. The important caveat is that this is only a test-tube finding so far, never confirmed in real patients, so I wouldn't panic. Still, before adding this supplement, please check in with your pharmacist or doctor.

“Take real care with supplements while on fluphenazine (Prolixin), an antipsychotic used mainly for schizophrenia. Five of the 148 ingredients on file rate major: herbs like henbane that stack anticholinergic side effects (dry mouth, blurred vision, confusion) onto a drug already prone to them, two rauwolfia plants that can intensify it…”

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain.
