Why this applies to Glipizide

Glipizide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Your body uses an enzyme called CYP2C9 to break down and clear glipizide. Lab research suggests plumbagin, a compound in Ceylon Leadwort, can slow that enzyme down. If that happened in real life, glipizide could hang around longer and act more strongly than expected.

For a blood-sugar medicine, that mostly means a higher chance of low blood sugar (hypoglycemia), with symptoms like shakiness, sweating, dizziness, confusion, or feeling suddenly hungry or weak. Here is the reassuring part: so far this is only a test-tube finding and has never been reported in people, so we honestly do not know how big the effect is. Still, since both affect blood sugar, please check with your pharmacist or doctor before combining them, and don't stop either one on your own.

Ceylon leadwort root extract has been shown to both increase and decrease blood glucose levels in animal research. This effect has not yet been demonstrated in humans. Theoretically, Ceylon leadwort might reduce or potentiate the effects of antidiabetes drugs. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the honest picture: in animal studies, Ceylon Leadwort root has pushed blood sugar in both directions, sometimes lower and sometimes higher. We don't have human studies confirming this yet, so this is more of a theoretical heads-up than a proven problem. Glipizide already works by squeezing more insulin out of your pancreas, so it lowers blood sugar on its own.

If the supplement also lowers blood sugar, the two together could drop you too low. That can show up as shakiness, sweating, dizziness, confusion, or a racing heart. On the flip side, if the leadwort raises your sugar, your glipizide might seem to work less well. Either way, the smart move is steady blood sugar checks and a quick chat with your pharmacist or doctor before mixing them. Don't change your glipizide dose on your own.

Glipizide tablets and glipizide extended-release tablets are both used as an add-on to diet and exercise to improve blood sugar (glucose) control in adults with type 2 diabetes. It is not used for type 1 diabetes or diabetic ketoacidosis (a serious complication of uncontrolled blood sugar).
