Why this applies to Hydrochlorothiazide, Pindolol

Hydrochlorothiazide, Pindolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

This combination pill pairs a water pill (hydrochlorothiazide) with pindolol, a beta-blocker. Pindolol is the part that matters here, since it's partly cleared by a liver enzyme called CYP2D6. In lab dishes, the plumbagin in Ceylon Leadwort has been shown to slow that enzyme down. If the same thing happened in the body, pindolol could build up a bit and hit harder than expected.

What that might feel like: a slower or pounding-feeling heartbeat, lower blood pressure, dizziness, or unusual tiredness. Keep in mind this is only a test-tube finding, never confirmed in real people, so the actual risk is uncertain. Still, since blood pressure and heart rate are involved, please check with your pharmacist or doctor before pairing them.
