Why this applies to Lacosamide

Lacosamide falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist with Vimpat (lacosamide) and Ceylon Leadwort. The plant compound plumbagin can block an enzyme called CYP2C9, which is one of the liver pathways that helps clear lacosamide. If that pathway slows down, lacosamide could build up a bit more than usual, which may mean stronger effects and more side effects like dizziness, drowsiness, blurred or double vision, headache, or unsteadiness.

I want to be honest about the evidence though: this is only seen in lab (test tube) studies, and it has not been reported in real people. So it's more of a theoretical caution than a proven problem. Still, since lacosamide is a seizure medicine, it's worth a quick chat with your pharmacist or doctor before adding this supplement. Don't change your dose on your own.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Lacosamide Used For?

Brand names: Vimpat, Motpoly XR

Lacosamide is used to treat partial-onset seizures (seizures that start in one part of the brain) in adults and children. The age at which it is approved depends on the specific product: Vimpat tablets, oral solution, and injection are approved for partial-onset seizures in patients as young as 1 month old; most other formulations are approved from age 4 and up; and Motpoly XR is approved for patients weighing at least 50 kg.

Lacosamide is also used as adjunctive (add-on) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain) in adults and children 4 years of age and older, including Motpoly XR for patients weighing at least 50 kg.
