Losartan Potassium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's a wrinkle worth knowing about losartan. Your body actually has to convert losartan into its more active form, and the enzyme CYP2C9 does a big part of that work. In lab studies, plumbagin (a compound in Ceylon Leadwort) appears to block CYP2C9. If that same blocking happened in people, it could mean less of the active form is made, so your blood pressure medicine may not work quite as well.

In plain terms, your blood pressure could creep up if the drug is weakened. That said, this is based only on test-tube research, not real human reports, so the actual risk is unclear. Before adding this supplement, please check with your pharmacist or doctor, and keep an eye on your blood pressure readings.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's a wrinkle worth knowing: losartan is actually a prodrug. Your liver uses an enzyme called CYP3A4 (along with CYP2C9) to turn it into its more powerful active form, which does most of the blood-pressure lowering. Ceylon Leadwort contains plumbagin, which in lab tests can slow down CYP3A4.

If that happened in a real person, it could mean less of losartan gets converted, so your blood pressure medicine might work a little weaker than expected. The catch is that this is only test-tube evidence (no human reports yet), so it's theoretical. Still, since blood pressure control matters, I'd run any Ceylon Leadwort supplement past your pharmacist or doctor first and keep an eye on your readings.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
