Why this applies to Piroxicam

Piroxicam falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist for Feldene (piroxicam): your liver relies on an enzyme called CYP2C9 to break this medicine down and clear it from your body. A compound in Ceylon Leadwort, plumbagin, has been shown to slow that same enzyme down, at least in lab dish studies. If it does the same thing in people, piroxicam could clear more slowly and build up a bit higher than usual.

With more piroxicam hanging around, you might see more of its typical NSAID side effects, things like stomach upset, heartburn, ulcers or bleeding, fluid retention, or kidney strain. Honestly though, this is only theoretical right now and hasn't been reported in real patients. I wouldn't panic, but do run it past your pharmacist or doctor before combining the two.

Piroxicam also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

There is some concern that Ceylon leadwort might potentiate the effects of anticoagulant and antiplatelet drugs and possibly increase the risk of bleeding. Ceylon leadwort root extract has been shown to decrease platelet adhesion and prolong bleeding in animals. However, this effect has not yet been demonstrated in humans. Until more is known, use cautiously in patients taking anticoagulant or antiplatelet drugs. Some of these drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.

Here's the thing to keep in mind with this pairing. Piroxicam is an NSAID, and on its own it already nudges up bleeding risk because it affects how your platelets stick together and can irritate the stomach lining. Ceylon Leadwort has shown, in lab and animal studies, that it can slow platelet clumping and lengthen bleeding time too. Stack the two and you could, in theory, get more bruising, nosebleeds, or stomach bleeding than either would cause alone.

That said, this hasn't actually been proven in people yet, so I don't want to overstate it. Still, since both lean the same direction, I'd play it safe. Talk with your pharmacist or doctor before combining them, especially if you take piroxicam daily or notice any unusual bruising or dark stools.
